Warranty Information and Client Consent
Press-On Veneers™ is a new lab direct dental technique designed for the purpose of cosmetic smile enhancement without a dental visit. Our process is designed to cover dark, stained, crooked, misaligned, or missing teeth. While the materials used for this process frequently appear safe, infrequent side effects can occur. Tooth and gum sensitivity or soreness is the most common side effect. Soft tissue irritation can also occur. If a client experiences these or other adverse symptoms, he/she should stop using the Press-On Veneer™, and consult their treating dentist or doctor. If the client has any known chemical sensitivity or allergic reactions to any dental ingredients please be sure to inform us directly prior to placing your order with us.
Press-On Veneer™ Limited Warranty Information
This warranty covers any defects in materials or workmanship in the Press-On Veneers® appliance, and runs for one (1) year from the date the client purchases the Press-On Veneers™.
Warranty covers:
Brighter Image Lab® will repair or replace a Press-On Veneers™ that proves to be defective in materials or workmanship. In some cases, new Press-On Veneers™ may need to be manufactured either from the existing impression or from a new impression. The repaired or replacement Press-On Veneers™ will be covered under the original warranty for the remaining time, if any, of the warranty period.
If your Press-On Veneers™ becomes damaged during the warranty period, and the conditions set forth in this warranty have been met and no exclusion applies, return the appliance to us for diagnosis. You can contact Brighter Image Lab® at 1-888-257-2455.
There will be no handling charge for warranty work for the first 90 days of the warranty period. During this initial 90 day period, there will be no charge for repair or replacement and return freight. After 90 days, there will be a $ handling fee (US Dollars) for all warranty claims, which will include return shipping.
Conditions that must be met for warranty to apply:
- To obtain warranty service, Clients will need a Return Authorization Number from Brighter Image Lab®. This can be obtained by calling 1-888-257-2455, or by emailing warranty@pressonveneers.com. If emailing, please provide a description of the problem.
- If the client purchased the Press-On Veneers™ directly from Brighter Image Lab® on credit terms, payments must be current to receive warranty service.
Warranty does not cover:
- Cash refunds are not covered under this warranty. No refund will be made after the client order is received.
- Changing shade from the original shade. No shade change will be made in any warranty claim for any reason.
- Modifying the teeth numbers from the original appliance.
- Damage or defects resulting from accident, neglect, abuse, failure of supportive tooth or tissue structures, improper insertion, improper adjustments, or improper oral hygiene.
- Incidental or consequential damages, exemplary damages, including inconvenience, lost wages or pain and suffering.
- Claims resulting from modifications made by the client to the appliance.
- Any other fees not covered unless specifically approved in writing in advance by Brighter Image Lab®.

Optional "No Fault" Extended Limited Warranty:
For $ (US Dollars), a "No Fault" Extended Limited Warranty may be purchased that provides repair or replacement of your Press-On Veneers™ for three (3) years (two additional years beyond the standard Limited Warranty). This extended coverage means Brighter Image Lab® will repair or replace your damaged Press-On Veneer™, no questions asked, for any reason whatsoever, even if the damage is the client's fault. All other terms and conditions of the Press-On Veneers™ Limited Warranty apply. This offer is valid for 30 days after receipt of appliance.

Optional "RUSH PROCESSING" Smile Express™
For $ (US Dollars), a "rush processing" order may be issued which provides top priority for the Press On Veneers™. This "rush processing" order means that Brighter Image® will make their "best effort" to ensure your Press On Veneers™ will arrive 12 business days from the date after proper impressions are received, however, the date of production, design, and delivery is never guaranteed.
The 12 business days excludes all weekend days and holidays and excludes shipping time. In the event the impressions Brighter Image® receives are not suitable to create the Press On Veneers™, a new set of veneers will be required and the 12 business days will reset on the day after Brighter Image® Lab receives a suitable set of the client's impressions.
Smile Express™ is only available to clients who pay the total amount for Press On Veneers™ and Smile Express™ at the time the client orders.
Smile Express™ is NOT a delivery time guarantee. Smile Express™ is an optional expedited request to be given top priority and to have your case move to the front of the production and design processes only.

CLIENT CONSENT
I hereby request and authorize Brighter Image Dental Lab®("Dental Lab") and whomever they may designate as their assistant(s), to build and perform (from an impression that I take myself) a custom Press-On Veneer™ made to fit over my existing teeth for cosmetic appearance only.
- I, on behalf of myself and my heirs, executors and administrators hereby release any and all claims, rights, liabilities and causes of action, known or unknown, that I may have against the above Brighter Image Dental Lab® and his/her staff as well as M.S.C. Development Inc. and its officers, directors, employees, successors and assigns arising from or related to the manufacture, supply, development and installation of a custom made dental appliance known as the Press On Veneers™ for my teeth and any other procedure(s) that the Dental Lab performs in connection therewith.
- I authorize the performance of additional procedures and changes of planned procedures if, in the judgment of "Dental Lab", this will be necessary to improve my safety and the results contemplated by the procedure.
- I understand that, although unusual, unexpected complications or less than desired results can occur. . I understand the “Dental Lab” is strictly making my press on veneers to fit the model of the impression(s) I provided and that I may not be satisfied with the final result.
- I understand that this process may between 30 and 90 days and requires me to return (Both) upper and lower impressions of my teeth along with (3) Three separate digital photos of my teeth from the front, back and side views before the lab process can start.
- I understand that excellent home care techniques, using a variety of aids, may add considerably to the successful outcome of my dental restorative device, and I understand it will be important for me to follow the home care instructions very carefully. I also understand that the Press On Veneers™ dental appliance is to be removed before sleeping for my personal safety as well as to extend the life of the product.
- I understand that since this appliance is built to fit the impressions I supply and requires care and maintence, and that the manufacturer cannot control my actions, I AGREE to INDEMNIFY AND HOLD HARMLESS the manufacturer and its dealers for any damages which result from your use or misuse of this dental appliance. I accept responsibility for any use and, hereby, waive right to claim loss or damage from its use.
- I understand that if I have selected to make payments on my Press-On Veneers™, I will make the first payment plus any shipping and handling costs at the time the order is placed. The remaining payment(s) will be charged each month to the credit card you've provided, beginning 30 days after the first charge and every 30 days thereafter until the completion of your payment plan. If you so desire, you can always pay-off your remaining payment(s) early with no penalty by contacting client services.
- I understand that effort will be made to make my teeth appear as straight as possible with Press On Veneers™, but because of the existing position of my natural teeth, which are not in perfect alignment, there is no results satisfaction guarantee that this can be accomplished. In addition, I understand there is NO Money Back Refunds and that price I am paying is to have my Press-On Veneers built to the best of the dental lab efforts. I also understand that the Press On Veneers™ appliance will add bulk to my teeth and that I will need to get used to the addition of the Press On Veneers™ to my teeth. By signing this informed consent form, I am confirming that I have read the above prior to my online agreement signature and understand this document and the proposed work in full, including its possible risks, complications, and benefits, and that I consent to, and authorize Brighter Image Dental Lab® to proceed with shipping my order and with, the necessary lab work as proposed, I understand that by placing my order I am ultimately responsible for all payment(s) and assume all other cost related to any collections cost.
- Jurisdiction of this Agreement is governed by and shall be construed in accordance with the laws of the State of Texas. The Parties agree to submit to the exclusive jurisdiction of the courts in Fort Worth, TX. for all purposes relating to this Agreement.
